In Washington (like nearly every state), couples therapy is typically NOT covered by insurance — because insurance pays for treating diagnosed mental disorders, and 'relationship problems' isn't one.

The workaround
If one partner has a diagnosable condition (depression, anxiety, PTSD) and the couples work is part of treating it, some plans will cover it. Your therapist would bill under the diagnosed partner's name with CPT code 90847 (family/couples therapy with patient).
Self-pay reality
Most couples therapy in {N} runs $150-300 per session, self-pay. Some therapists offer sliding scale. EFT-trained couples therapists tend to be at the higher end; newer therapists often at the lower end.
